Payment & Cancellation Policy One on One Consults
Forms of Payment Accepted: Debit and most major credit cards via Square and/or Paypal
Online Services: First payment is due at the time of scheduling your appointment.
In Person: First payment is due at the end of your first appointment.
Payment plans are available for all Nutritional Therapy Follow Up packages. The total will be divided into 2 or 3 payments (depending on the length of package) and a 5.0% transaction fee will be applied. You will be charged at your Functional Body Evaluation and then again at your 1st follow up.
Nutritional supplements, herbs and essential oils may be recommended as part of your protocols. The price of these products is not included in your package and will be an additional cost.
Cancellation Policy: you must cancel 24 hours in advance as common courtesy to the practitioner and to other clients who may be able to attend during that time slot. Failure to cancel 24 hours prior to your appointment time (on acuity schedule or by email) will result in a $50 cancellation fee. This includes in person and online Initial Consults, Follow Ups and Functional Body Evaluations.
